Physicals
for 9th Graders and New Students
9th graders and all other students
new to the Hanover Park Regional High School District must complete a physical
examination by their private healthcare provider during the first year of attendance.
An examination conducted within 365 days of the start of school will be accepted
provided it is properly documented. Forms for the entry physical are available
from the school nurse. Students who do not submit proof of this will be scheduled
for a physical with the school physician.
Students currently in 9th grade who have had a physical for athletics need not have another physical. The same is true for students new to our district who have had an athletic physical.
