Athletic Physical Examinations Physical Forms
The following forms make up the Athletic Packet - this entire Packet must be completed once every 365 days along with a physical examination in order to participate in athletics. These forms are available at the school or can be downloaded by clicking on the links below.

Fall 2008 Sports Physicals

If you have not had a physical exam within 365 days, you must have a physical exam and submit a completed Athletic Packet no later than Wednesday, Aug. 13 in order to participate in spring sports. You may get a physical from your own physician or have an exam from the WP physician on the following days:

Boys ~ Wednesday, August 13
10:30 a.m.- 12:30 p.m.

Girls ~ Thursday, August 14
8:00 a.m. - 10:00 a.m.

Reminder - All forms must be approved by the WP school physician before an athlete can begin practice. To ensure that your child can begin practice on time, adhere to the deadlines established before the start of each season.

Athletes who have had a physical within 365 need to submit only a yellow form which includes a health history update, a blue athletic application and permission form and a Random Drug Testing Consent Form in order to participate. Forms are due no later than Wednesday, August 13.



Physicals for 9th Graders and New Students
9th graders and all other students new to the Hanover Park Regional High School District must complete a physical examination by their private healthcare provider during the first year of attendance. An examination conducted within 365 days of the start of school will be accepted provided it is properly documented. Forms for the entry physical are available from the school nurse. Students who do not submit proof of this will be scheduled for a physical with the school physician.

Students currently in 9th grade who have had a physical for athletics need not have another physical. The same is true for students new to our district who have had an athletic physical.
